Impression: This split night sleep study documented snoring and severe sleep apnea with a Respiratory Disturbance Index (RDI)* of 46 events/hour and an Apnea+Hypopnea Index (AHI) of 43 events/hour in the diagnostic section. There were O2 desaturations to 86%. The patient was given a trial of CPAP and improved with a CPAP setting of 8cm H2O which eliminated snoring and reduced the RDI and the AHI to four events/hour. The ECG demonstrated a sinus rhythm throughout the recording. The patient rated the night as being “average” when compared with a typical night at home.
Recommendation:
1. A home trial of CPAP at 8cm H2O with the medium Quattro FX full face mask and a heated humidifier.
2. Close clinical follow-up to ensure that the patient is tolerating and compliant with the treatment.
3. Weight loss.
4. Mr. Phillips should avoid driving if excessively sleepy
